Crease in serum ferritin concentration, which reflects the high demand that epoetin alfa therapy places on body iron stores. Finally, we omitted the dose of epoetin alfa on the day of surgery. In our opinion, dosing at this time is unlikely to be effective for prevention of transfusion, given that the majority of blood is transfused in the first 3 days after surgery. These considerations highlight an important limitation of this study. Because multiple alterations to the standard regimen were made, we cannot identify which one was primarily responsible for the enhanced treatment response. Further refinements to the regimen may result in even greater efficacy. Another potential limitation is that we did not establish guidelines for allogeneic transfusion at the participating centers. Nevertheless, transfusion practice during this trial reflected usual clinical practice. Which of the two epoetin alfa regimens studied in this trial is to be recommended? The reduction in transfusion was significant in both epoetin alfa groups compared with placebo. However, the transfusion rate was lowest in patients who received the high-dose regimen. Moreover, the increase in reticulocyte count and hemoglobin concentration was greatest in the high-dose group, and the multivariate model identified both of these variables as significant predictors of the occurrence of transfusion. Therefore, we speculate that the superior bone marrow response observed in the high-dose group might have accounted for the additional reduction in transfusion. The observed difference was not statistically significant 11.4% in the high-dose and levetiracetam.

Cryptosporidium is a minute coccidian parasite that causes selflimiting diarrhoea, usually in young children and immunocompromised people. The infective stage of Cryptosporidium is an oocyst containing sporozoites. The development of sporozoites occurs within the brush border of epithelial cells of the intestine. Sporulated oocysts that contain 4 sporozoites each are passed in the stools of infected patients. Using an immunofluorescent staining technique auramine ; , these are visualised as small round bodies, 24 m in size. In the diagnostic microbiology laboratory, all stool specimens from children under 6 years of age are routinely examined for the presence of Cryptosporidium oocyts. Stool specimens from patients 6 and over are only screened for Cryptosporidium at the request of the clinician or when clinical data indicates that the patient is immunocompromised. Data collected for a period of 1 year 2001 ; revealed the following results: Of the 183 specimens examined in children under the age of 6, 21 were positive 11.5 % ; . Of the 227 specimens examined in immunocompromised persons 6 and over, 14 were positive 6.2 % ; . Of the 21 positives found in children under 6, only 1 occurred in a child older than 1 year. These results reflect a high incidence of Cryptosporidium in children 1 year and younger, and suggests that routine screening in children above this age is not indicated. The relatively low incidence of Cryptosporidium in selected patients 6 years and older suggests that the selection criteria may need to be changed in order to yield a more cost-effective result.

Levamisole wormer for dogs
Background: The drug levamisole has been successfully used in combination with fluorouracil to increase the disease-free interval and survival of patients who have undergone surgical resection of Dukes' stage C colon cancer. Levamisole is thought to affect the host immune response. Several recent studies have examined its effect on the expression of major histocompatibility complex MHC ; class I molecules, but the results have been inconsistent. An equally important requirement for a host cellular immune response is the adhesion of leukocytes to tumor cells. The latter may be required for cell-mediated antitumor cytotoxic responses. Purpose: We evaluated the ability of levamisole to affect the expression of MHC class I molecules and cell-adhesion molecules and determined whether levamisole could affect leukocyte adhesion to tumor cells that had been treated with the drug. Methods: A panel of four human colon tumor cell lines HT-29, SW-620, HCT-15, and LoVo ; , A-375 human melanoma cells, and human umbilical vein endothelial cells HUVEC ; were cultured in the presence of levamisole and examined by solid-phase enzyme immunoassay to determine the level of expression of MHC class I, intercellular adhesion molecule 1 ICAM-1 ; , vascular cell-adhesion molecule-1 VCAM-1 ; , leukocyte integrin VLA-4, and lymphocyte-functional antigen LFA-1 ; molecules. Adhesion of HL-60 and THP-1 myeloid cells to tumor cells was also evaluated. Tumor necrosis factor TNF ; at 10 ng was used as a positive control for increasing adhesion molecule expression and cell-cell adhesion. The statistical significance of differences in cell surface molecule expression and functional adhesion between treated and control cells were tested by use of analysis of variance and the two-tailed Dunnett's test. Results: Treatment with levamisole 0.1 and 1 |ig mL ; caused the levels of MHC class I expression to increase approximately threefold above control levels on HCT-15 and LoVo colon tumor cells P .05 in each case ; compared with untreated cells, caused minimal increases on HT-29 cells to 1.5 times control levels ; , but caused no significant increases on SW-620 colon tumor or A-375 melanoma cells. The HCT15 and LoVo colon tumor cells had very low basal MHC expression. Levamisole 1 |ig mL ; increased VCAM-1 expression on HT-29 and SW-620 colon tumor cells to 4.3 and 2.4 times f .05 in each case ; control levels, respectively, doubled ICAM-1 expression on HT-29 cells P .0S ; , and increased LFA-1 expression on HT-29, LoVo, and A-375.
